ABSTRACT Background: Information is needed on the safety and efficacy of direct discharge from the emergency department (ED) of patients with COVID-19 pneumonia. Objectives: The objectives of the study were to study the variables associated with discharge from the ED in patients presenting with COVID-19 pneumonia, and study ED revisits related to COVID-19 at 30 days (EDR30d). Methods: Multicenter study of the SIESTA cohort including 1198 randomly selected COVID patients in 61 EDs of Spanish medical centers from March 1, 2020, to April 30, 2020. We collected baseline and related characteristics of the acute episode and calculated the adjusted odds ratios (aOR) for ED discharge. In addition, we analyzed the variables related to EDR30d in discharged patients. Results: We analyzed 859 patients presenting with COVID-19 pneumonia, 84 (9.8%) of whom were discharged from the ED. The variables independently associated with discharge were being a woman (aOR 1.890; 95%CI 1.176-3.037), age < 60 years (aOR 2.324; 95%CI 1.353-3.990), and lymphocyte count > 1200/mm3 (aOR 4.667; 95%CI 1.045-20.839). The EDR30d of the ED discharged group was 40.0%, being lower in women (aOR 0.368; 95%CI 0.142-0.953). A total of 130 hospitalized patients died (16.8%) as did two in the group discharged from the ED (2.4%) (OR 0.121; 95%CI 0.029-0.498). Conclusion: Discharge from the ED in patients with COVID-19 pneumonia was infrequent and was associated with few variables of the episode. The EDR30d was high, albeit with a low mortality.

Abstract This text focuses on different artistic/curatorial practices that, since the late 1980s, have proposed possible responses to the stagnation of institutions and their incapacity to dialogue -from the art system- with conjunctural socio-political processes in the territories of Abya Yala. We make an approach to these situated practices based on G. Pollock's notions of virtuality, D. Haraway's encounter and fabulation, and S. Rivera Cusicanqui's ch'ixi. In them we highlight different poetic/political operations that function as antecedents to propose an artistic/curatorial practice in the present, which we have called Hacer(nos) fuego. The aim of the project is to tension, among others, the following questions: What kind of artistic/curatorial operations can we expect today, in the context of social and pandemic outbursts? In what space-time and with what methodologies could artistic events take place and give rise to other epistemologies?
Resumen Este texto se detiene en distintas prácticas artísticas/curatoriales que, desde finales de los años ochenta, han propuesto posibles respuestas al anquilosamiento de las instituciones y su imposibilidad de dialogar -desde el sistema del arte- con procesos sociopolíticos coyunturales en territorios de Abya Yala. A partir de las nociones de virtualidad de G. Pollock, encuentro y fabulación de D. Haraway, y lo ch’ixi de S. Rivera Cusicanqui, nos aproximamos a estas prácticas situadas. En ellas destacamos distintas operaciones poéticas/políticas que funcionan como antecedentes para proponer una práctica artística/curatorial en el presente, que hemos llamado Hacer(nos) fuego. Esta busca tensionar, entre otras, las siguientes preguntas: ¿Qué tipo de operaciones artístico/curatoriales podemos pensar hoy, en el contexto de los estallidos sociales y pandémicos? ¿En qué espacio-tiempo y con qué metodologías pueden ocurrir acontecimientos artísticos que den lugar a otras epistemologías?

ABSTRACT Background: Periocular basal cell carcinoma is a tumor lesion arising from the epidermis and hair follicles basal cells, with a high potential local destruction, can be disfiguring and invade the surrounding tissue leading to deformities or loss of function of the affected organ. In order of appearance it is most common in the lower eyelid, medial edge, upper eyelid and temporal edge. Objective: To describe the results of the application of HeberFERON in a case series with periocular basal cell carcinoma who attended dermatology appointment at the Policlínico Centro, from January 2017 to December 2020. Methodology: A series study of clinical cases with periocular basal cell carcinoma who attended the dermatology appointment at the Policlínico Centro was conducted. 17 cases with clinical, dermatoscopic and histopathological diagnosis were included. A baseline evaluation was conducted, during and 16 weeks after treatment; 10.5 IU of HeberFERON was administered 3 times a week perilesional and intradermally until completing 9 doses. The main variables were the treatment response and the presence or absence of adverse events. Results: Male sex, phototypocutaneous II, lower eyelid location, clinical subtype ulcerative nodule and solid histological subtype predominated, complete response was achieved in most patients. Adverse events were pain at the injection site, fever, general malaise, edema and perilesional erythema. Conclusions: Treatment response was favorable in most patients treated with HeberFERON.
RESUMEN Fundamento: El carcinoma basocelular periocular es una lesión tumoral que surge de las células basales de la epidermis y los folículos pilosos, con un alto potencial de destrucción local, pueden ser desfigurantes e invaden el tejido que los rodea dando lugar a deformidades o pérdida de la función del órgano afectado. En orden de aparición es más común en el párpado inferior, el canto medial, el párpado superior y el canto temporal. Objetivo: Describir los resultados de la aplicación del HeberFERON en una serie de casos con carcinoma basocelular periocular que acudieron a consulta de dermatología del Policlínico Centro, de enero de 2017 a diciembre del 2020. Metodología: Se realizó un estudio de serie de casos clínicos con carcinoma basocelular periocular que acudieron a la consulta de dermatología del Policlínico Centro. Se incluyeron 17 casos con diagnóstico clínico, dermatoscópico e histopatológico. Se realizó una evaluación inicial, durante y 16 semanas después del tratamiento; se administró 10.5 UI de HeberFERON 3 veces por semana perilesional e intradérmica hasta completar 9 dosis. Las variables principales fueron la respuesta al tratamiento y la presencia o no de eventos adversos. Resultados: Predominó el sexo masculino, el fototipocutáneo II, la localización en párpado inferior, el subtipo clínico nódulo ulcerativo y el histológico sólido, se logró respuesta completa en la mayoría de los pacientes. Como eventos adversos se presentaron dolor en el sitio de inyección, fiebre, mal estar general, edema y eritema perilesional. Conclusiones: La respuesta al tratamiento fue favorable en la mayoría de los pacientes tratados con HeberFERON.

ABSTRACT The aim of this study was to determine the post-vaccine response of canines and felines of the municipality of San Sebastián de Mariquita, Colombia, which presented in 2014 a fatal case of canine rabies due to atypical bat variant. A cross-sectional and nonexperimental sampling by convenience was carried out on 130 canines and 38 felines from the rural and urban area of the municipality. Blood samples were taken. The anti-rabies antibody titration was done using the ELISA kit (Platelia Rabies II Kit Ad Usum Veterinarium) approved by the OIE for the detection and titration of IgG antibodies against the glycoprotein of the virus. The classification categories (protection status) were established as Protected (> 0.5 IU/ml), Insufficient protection (>0.1 <0.5 IU/ml) and Unprotected (<0.1 IU/ml). Results showed that 40% (52) of the canines obtained titles corresponding to the protected category, 59% (77) to the insufficient protection category and 1% (1) to the unprotected category. The felines presented a similar percentage distribution among the categories: protected 34% (13), insufficient protection 32% (12) and unprotected 34% (13).
RESUMEN El objetivo de la investigación fue determinar la respuesta posvacunal de caninos y felinos del municipio de San Sebastián de Mariquita, Colombia, que presentó en 2014 un caso fatal de rabia canina por variante atípica de murciélago. Se realizó un muestreo por conveniencia, de corte transversal y no experimental a 130 caninos y 38 felinos de la zona rural y urbana del municipio citado. Se tomaron muestras de sangre y para la titulación de anticuerpos antirrábicos se utilizó un kit de ELISA (Platelia Rabies II Kit Ad Usum Veterinarium) aprobado por la OIE para la detección y titulación de anticuerpos Ig G contra la glicoproteína del virus. Las categorías de clasificación (estado de protección) fueron establecidas como Protegido (>0.5 UI/ml), Insuficiente protección (>0.1 <0.5 UI/ml) y Sin protección (<0.1 IU/ml). El 40% (52) de los caninos obtuvieron títulos correspondientes a la categoría protegidos, 59% (77) insuficiente protección y 1% (1) sin protección. Los felinos presentaron una distribución porcentual similar entre las categorías: protegido 34% (13), insuficiente protección 32% (12) y sin protección 34% (13).

ABSTRACT Objective: to provide evidence based clinical recommendations for the diagnosis and therapy of the acute stage of ischemic cerebrovascular accident in EsSalud. Materials and methods : a group for producing the guideline was conveyed, including specialized physicians and methodologists. The group prepared 8 clinical questions to be answered by the guideline. Systematic searches of previous reviews were performed, and - when it was deemed necessary - primary studies in Medline and the Cochrane Controlled Registry of Trials for 2018 were reviewed. Evidence was selected aiming to respond each one of the proposed clinical questions. Certainty of the evidence was determined using the Grading of Recommendations Assessment, Development, and Evaluation (GRADE) methodology. In periodic work sessions, the group used GRADE methodology for reviewing the evidence and generated recommendations, good clinical practice items, and designed flow charts for both diagnosis and therapy. Finally, the guide was approved with the decree N° 128-IETSI-ESSALUD-2019. Results : this clinical practice guideline focused in 8 clinical questions, which were divided into four topics: screening, diagnosis, therapy, support and rehabilitation. On the basis of these questions, 28 recommendations were formulated (8 strong and 20 conditional), 38 items for good clinical practice, 1 implementation note, and 2 flow charts. Conclusion : this paper summarizes both evidence based methodology and conclusions from a new clinical practice guide for diagnosis and therapy of the acute stage of a cerebrovascular accident in EsSalud.
RESUMEN Objetivo: proveer recomendaciones clínicas basadas en evidencia para el diagnóstico y tratamiento de la etapa aguda del ataque cerebrovascular isquémico en EsSalud. Materiales y métodos: se conformó un grupo elaborador de la guía (GEG) que incluyó médicos especialistas y metodólogos. El GEG formuló 8 preguntas clínicas a ser respondidas por la presente GPC. Se realizó búsquedas sistemáticas de revisiones sistemáticas y -cuando fue considerado pertinente- estudios primarios en Medline y Cochrane Controlled Register of Trials durante el 2018. Se seleccionó la evidencia para responder cada una de las preguntas clínicas planteadas. La certeza de la evidencia fue evaluada usando la metodología Grading of Recommendations Assessment, Development, and Evaluation (GRADE). En reuniones de trabajo periódicas, el GEG usó la metodología GRADE para revisar la evidencia y formular las recomendaciones, los puntos de buenas prácticas clínicas y el flujograma de diagnóstico y tratamiento. Finalmente, la GPC fue aprobada con Resolución N° 128-IETSI-ESSALUD-2019. Resultados: la presente GPC abordó 8 preguntas clínicas, divididas en cuatro temas: tamizaje, diagnóstico, tratamiento, soporte y rehabilitación. En base a estas preguntas se formularon 28 recomendaciones (8 fuertes y 20 condicionales), 38 puntos de buena práctica clínica, 1 nota de implementación y 2 flujogramas. Conclusión: el presente artículo resume la metodología y las conclusiones basadas en evidencias de la GPC para el diagnóstico y tratamiento de la etapa aguda del ataque cerebrovascular isquémico en EsSalud.

Teleconsulta como sistema de información en el cuidado de pacientes con deterioro de la integridad cutánea

ABSTRACT Objectives: To evaluate the benefits of teleconsultation compared to the conventional face-to-face consultation in patients with cutaneous integrity deterioration. Methodology: Pilot study of controlled, randomized, open intervention. Subjects of study: patients with deterioration of the cutaneous integrity belonging to the urban health centers. Two groups were created, one on which the intervention will be carried out through teleconsultation and another control group where the consultation and Assessment was done in person. Collection of data through observation, interviews, questionnaires and validated scales. Results: A total of 31 consultations were carried out from the health centers, 12 of which were in person (38.7%) and 19 through teleconsultation (61.3%). The lesions consulted are mostly of venous origin. These patients present altered quality of life. A mean improvement of 6 points is obtained in the PUSH scale in the face-to-face modality versus 8 points in the teleconsultation, the time in which the epithelialization of the lesions in the latter is achieved. Conclusions: Teleconsultation is emerging as a new organizational system, a new way of organizing and managing the provision of health services for the benefit of patients, professionals and the health system in general. Establishing a fast, fluid, effective and efficient communication channel that has a direct impact on the patient, reducing time for the resolution of his health problem, avoiding unnecessary movements and reducing costs.
RESUMEN Objetivos: Evaluar los beneficios de la teleconsulta frente a la consulta presencial convencional en pacientes con deterioro de la integridad cutánea. Metodología: Estudio piloto de intervención controlado, aleatorizado, abierto. Sujetos de estudio: pacientes con deterioro de la integridad cutánea pertenecientes a los centros de salud urbanos. Se crearon dos grupos, uno sobre el que se realizó la intervención a través de la teleconsulta y otro grupo control donde la consulta y la valoración se hizo de forma presencial. Recogida de los datos mediante observación, entrevistas, cuestionarios y escalas validadas. Resultados: Se realizaron un total de 31 consultas desde los centros de salud, 12 de las cuales fueron de forma presencial (38,7%) y 19 a través de la teleconsulta (61,3%). Las lesiones consultadas fueron en su mayoría de etiología venosa. Estos pacientes presentaban alteración de su calidad de vida. Se obtuvo una media de mejoría de 6 puntos en la escala PUSH en la modalidad presencial frente a 8 puntos en la teleconsulta, siendo menor el tiempo en que se consigue la epitelización de las lesiones en esta última. Conclusiones: La teleconsulta se perfila como un nuevo sistema organizativo, una nueva manera de organizar y gestionar la provisión de los servicios sanitarios en beneficio de los pacientes, profesionales y sistema sanitario en general, estableciendo un canal de comunicación rápido, fluido, efectivo y eficiente que repercuta directamente en el paciente disminuyendo tiempos para la resolución de su problema de salud, evitando desplazamientos innecesarios y disminuyendo los costes.

Background: as part of the research project «Outstanding Educators of the 20th Century», the personality of Dr. Norma Ciriaca Pérez Calleja, pediatrician and outstanding professor from Ciego de Ávila province was selected for this study. Objective: to let know the life and work of the aforementioned doctor as a paradigm in the field of Pediatrics and medical education. Methods: a descriptive research work within the qualitative approach was carried out at Ciego de Ávila University of Medical Sciences, in the academic year 2016-2017. The biographical or life history method was used to reconstruct the vital development as a singular expression of the social reality lived by the doctor, through different sources: documentary review of her teaching file and other materials that endorse her social recognitions, chronicles about her life, scientific works, interviews, testimonies of colleagues and patients, and photos. Results: the life of the doctor and professor was characterized and her contribution to the training of professionals of the medical sciences in Ciego de Ávila. The documents consulted allowed to endorse her social behavior as a living example, her firm convictions and strong will, all of which make her a bearer of high spiritual principles and values, which are the guide for many others and become significant teachings for the majority of those who know her. Conclusions: the mobilizing influence, professionalism and educational work of Dr. Norma Ciriaca Pérez Calleja allowed considering her as a social educator and an example to follow for the new generations.
Fundamento: como parte del proyecto de investigación «Educadores Destacados del Siglo XX», se seleccionó para su estudio la personalidad de la Dra. Norma Ciríaca Pérez Calleja, pediatra y profesora ilustre de la provincia Ciego de Ávila. Objetivo: divulgar la vida y obra de la mencionada doctora como paradigma en el campo de la Pediatría y la educación médica. Métodos: se realizó una investigación descriptiva con enfoque cualitativo en la Universidad de Ciencias Médicas de Ciego de Ávila, en el periodo 2016-2017. Se utilizó el método biográfico o historia de vida para reconstruir el desarrollo vital como expresión singular de la realidad social vivida por la doctora, a través de distintas fuentes: revisión documental de su expediente docente y otros materiales que avalan sus reconocimientos sociales, crónicas sobre su vida, trabajos científicos, entrevistas, testimonios de colegas y pacientes, y fotos. Resultados: se caracterizó la vida de la doctora y profesora y su aporte a la formación de los profesionales de las ciencias médicas en Ciego de Ávila. Los documentos consultados permitieron avalar su conducta social como ejemplo vivo, sus firmes convicciones y fuerte voluntad, todo lo cual la hacen portadora de elevados principios y valores espirituales, los que constituyen guía de muchos y devienen significativas enseñanzas para la mayoría de los que la conocen. Conclusiones: la influencia movilizadora, profesionalidad y labor educativa de la doctora Norma Ciriaca Pérez Calleja permitieron considerarla como una educadora social y ejemplo a seguir por las nuevas generaciones.

ABSTRACT INTRODUCTION Iron deficiency is the main cause of anemia, but infections, inflammation and other factors also play a role. Anemia in women of childbearing age is a risk for pregnancy, childbirth and childhood development during the first two years of life. In Cuba, per WHO definition, anemia is a moderate public health problem in the third trimester of pregnancy and in preschoolers, with a prevalence of 21.6%, in both cases. OBJECTIVE Estimate prevalence of anemia and iron deficiency in women of childbearing age and assess its relation to inflammation, overweight, central adiposity, H. pylori infection and ingestion of iron-rich foods and enhancers of iron absorption. METHODS A cross-sectional, analytical study was performed in 391 women aged 18–40 years in four municipalities of Havana, Cuba, from February through June 2014. Variables (indicators in parentheses) were anemia (hemoglobin), iron deficiency (ferritin), nutritional status (body mass index and waist circumference), inflammation (C-reactive protein, acid alpha 1-glycoprotein and interleukin 6), H. pylori infection and ingestion of iron-rich foods. SPSS 20.0 and Epi Info 7.1.2.0 were used for statistical analysis. RESULTS Anemia prevalence was 24.6% (96/391); iron deficiency, 68% (266/391); H. pylori infection, 47.1% (184/391); inflammation detected by C-reactive protein, 8.4 % (33/391) and by alpha-1-gly-coprotein, 19.9% (78/391). Limited results are included for interleukin 6, which was determined in fewer cases (96). Excess body weight was found in 38.7% (150/388) and increased central adiposity in 26.7% (101/378). Iron deficiency was the main cause of anemia (OR 2.68). Central adiposity, excess body weight, and iron deficiency were positively associated with inflammation (OR of 1.77, 1.23 and 1.72, respectively), whereas H. pylori infection was negatively associated with iron deficiency and anemia (OR 0.75 and 0.94, respectively). Low consumption of meat (OR 1.17) and vegetables (OR 1.36) showed discrete limited positive associations with iron deficiency, as well as low consumption of eggs (OR 1.69) and vegetables (OR 1.56) with anemia. CONCLUSIONS Anemia is a moderate public health problem in the studied group, but with iron deficiency present in two thirds of the population and associated with anemia. Risk factors for anemia and iron deficiency, such as menorrhagia and bacterial or viral infections, should be assessed in women of childbearing age, to support interventions needed to reduce risks in pregnancy and childbirth.

Abstract Introduction: Abdominal fat and its increment over time in particular has become a cardiovascular risk factor in uraemic patients. Objectives: To analyse changes in abdominal fat in haemodialysis patients over one year and study their possible correlation with the variation in adipocytokine serum levels. As a secondary objective, we tried to validate the data obtained by bioelectrical impedance analysis (BIA) with data obtained by dual X-ray absorptiometry (DXA). Material and methods: A prospective one-year study was performed in 18 patients on haemodialysis (HD). In each patient, body composition by BIA and DXA was estimated at baseline and after one year. Several adipocytokine and biochemical parameters were determined. Results: A significant increase in phase angle [4.8° (4.1-5.6) vs. 5.2° (4.4-5.8), P < .05], BIA intracellular water [48.3% (43.1-52.3) vs. 50.3% (45.7-53.4), P < .05] and the ratio between the percentage of android/gynecoid (A/G) distribution of fat measured by DXA [1.00 (0.80-1.26) vs. 1.02 (0.91-1.30), P < .05] was observed. A statistically significant relationship between leptin and adiponectin concentrations and the percentage of fat mass measured by BIA, as well as the abdominal fat percentage estimated by DXA, was found (P < .01). Conclusion: HD patients exhibit a gain in fat mass over time, especially in the abdomen, evidenced by an increased A/G ratio. These findings might explain the increased cardiovascular risk in these patients.
Resumen Introducción: La grasa abdominal y, sobre todo, su ganancia a lo largo del tiempo, se ha consolidado como un factor de riesgo cardiovascular en pacientes urémicos. Objetivos: Analizar los cambios en la grasa abdominal en los pacientes de hemodiálisis (HD) a lo largo de un año y estudiar sus posibles relaciones con los cambios en los niveles circulantes de adipocitocinas. Como objetivo secundario intentamos validar los datos obtenidos por bioimpedancia eléctrica (BIA) con los obtenidos por absorciometría dual de rayos X (DXA). Material y métodos: Se realizó un estudio prospectivo de un año de duración en 18 pacientes en HD. En cada paciente se cuantificó, basalmente y al cabo de un año, la composición corporal por BIA y DXA y se determinaron varios parámetros bioquímicos incluyendo adipocitocinas. Resultados: Se evidenció un aumento significativo del ángulo de fase [4,8° (4,1-5,6) frente a 5,2° (4,4-5,8); p < 0,05], del agua intracelular por BIA [48,3% (43,1-52,3) frente a 50,3% (45,7-53,4); p < 0,05] y del cociente entre el porcentaje de grasa de distribución androide/ginecoide (A/G) medido por DXA [1,00 (0,80-1,26) frente a 1,02 (0,91; 1,30); p < 0,05]. Se encontró una relación estadísticamente significativa entre las concentraciones de leptina y adiponectina tanto con el porcentaje de masa grasa medida por BIA como con la grasa abdominal estimada mediante DXA (p < 0,01). Conclusión: Los pacientes en HD experimentan una ganancia de grasa con el tiempo, especialmente en localización abdominal, evidenciada por un aumento del cociente A/G, lo que podría explicar el aumento del riesgo cardiovascular que presentan.

Introduction: prosthetic rehabilitation extends beyond the individual tooth or the single area without teeth, which includes restoring the normality of the relations of muscles and teeth, as well as the proper oral health, substituting the teeth absent by artificial means and achieving the masticatory, aesthetics and phonetics function. Objective: to identify the real need for stomatology prosthesis in patients older than 15 years of René Vallejo Ortiz University Polyclinic III Method: a cross-sectional descriptive study was done. Of a total of 19.760 individuals, 500 of them were randomly selected, representing the sample. The oral examination was performed considering the informed consent of all patients. Results: the female sex represented 60% and the group of 35-59 years 45%; 56% of the patients needed treatment, partial dentures were the most needed with 62.5%, the female sex showed a real need of 64.3% and the felt need for the prosthesis represented 51.8%, higher in the female sex with 68.9% and in the age group between 35-59 (56.7%). Conclusions: more than half of the population required partial-type prosthetic treatment, the real and felt need for treatment was more evident in the female sex and in the age group between 35-59.
Introducción: la rehabilitación protésica se extiende más allá del diente individual o del área desdentada única, comprende la restauración de la normalidad de las relaciones de los músculos y dientes, de modo que se devuelva la salud bucal, tiene como objetivo la sustitución de los dientes, cuando se encuentran perdidos o ausentes, por medios artificiales capaces de restablecer la función masticatoria, estética y fonética. Objetivos: identificar la necesidad real y sentida de prótesis estomatológica en pacientes mayores de 15 años del Policlínico Universitario III René Vallejo Ortiz. Métodos: se realizó un estudio descriptivo transversal donde del universo de 19 760 personas se examinaron 500 personas que representaron la muestra la cual se escogió de forma aleatoria; se realizó el examen bucal y formulario, se contó con el consentimiento de todos los pacientes seleccionados. Resultados: el sexo femenino representó el 60% de los examinados y el grupo de 35-59 años el 45%. El 56% de los examinados necesitó tratamiento, las prótesis parciales fueron las más necesitadas con 62,5%, el sexo femenino mostró una necesidad real del 64,3% y la necesidad sentida de prótesis estomatológica fue del 51,8%, mayor en el sexo femenino con el 68,9% y en el grupo de edad de 35-59 años con el 56,7%. Conclusiones: más de la mitad de la población examinada necesitó tratamiento protésico de tipo parcial, la necesidad real y sentida de tratamiento fue más notoria en el sexo femenino y en el grupo de edad de 35 a 59 años.
